About the role
Key responsibilities & impact- Provide client support and case management across clinical, community, detention, and harm reduction settings
- Conduct intakes and enrollment activities
- Follow care plans for clients
- Complete data tracking and monthly reports
- Document appropriately in the electronic health record
- Accompany clients to meetings
- Participate in community group meetings to advocate for client needs
- Work Monday–Friday, 8:00 a.m.–5:00 p.m., for 30–40 hours per week
Requirements
What you’ll need- Bachelor's degree in a human service field or similar degree with two years of experience with incarceration, probation, substance use recovery, mental health treatment, or homelessness OR high school diploma or equivalent with 4 years of experience in these areas and a peer support specialist certification OR equivalent education or experience
- Knowledge of harm reduction principles, recovery, and person-first language
- Ability to plan and work collaboratively with community partners and CHA staff in person and virtually
- Ability to work with diverse groups of people and deal tactfully with others
- Ability to exercise good judgment in appraising situations and making decisions
- Ability to accurately record services rendered and interpret and explain records
- Lived experience preferred
