About the role
Key responsibilities & impact- Assess potential new members for program eligibility and explain the program to members and families
- Conduct community outreach as needed
- Perform initial assessments using the UAS (Uniform Assessment System) tool
- Evaluate potential members for eligibility and safety
- Determine care needs and formulate initial plans of care
- Communicate with clinical teams to ensure seamless service delivery
- Complete enrollment documentation
- Serve as a community resource for intake issues
- Integrate age-specific requirements with geriatric members' developmental, mental, social, and cultural needs
- Communicate with potential members, families, referral sources, and physicians to support transitions to managed long-term care
- Collaborate with Intake/Outreach and clinical teams
- Travel regularly to homes, offices, and other public settings
Requirements
What you’ll need- Associate degree in Nursing required
- One (1) year experience working in an acute care setting required
- Current Registered Nurse license in the state of New York required
- Must be able to read, write, and speak English
- Knowledge of Medicare and Medicaid regulations
- Must have valid driver's license, vehicle, and verifiable insurance
- Successful clearance of a driver's license record check required
- Annual influenza vaccination required during influenza season (October 1–March 31)
- Must reside in the same territory assigned to work in, unless an exception is considered due to business need
- Ability to travel greater than 50% of the time
- Ability to use general office equipment
- Bachelor's degree, home care experience, and MLTC-intake experience preferred
