About the role
Key responsibilities & impact- Provide information and assistance linking participants with medical, social and/or educational providers and other programs and services
- Address identified needs and help participants achieve identified goals
- Conduct health and safety, social and environmental assessments according to established guidelines
- Serve on work groups, community groups, committees or task forces
- Attend meetings as requested
- Work directly with individuals to assess needs, develop personalized care plans and connect them with essential services and supports
Requirements
What you’ll need- Bachelor’s degree in a human service field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing, or a closely related field
- Two years’ direct service experience serving persons with DD/IDD or related disabilities
- Two years’ experience in a healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting
- Experience completing assessments, service plan development and monitoring
- Effective organizational and communication skills
- Experience providing care management/coordination preferred
- Willingness and ability to work a flexible schedule to meet the needs of families
- Reliable transportation
- Valid driver’s license and car insurance as required by state law
- Ability to travel
- Adherence to the CareStar Rule
- Compliance with CareStar Policies and Procedures
- Ability to maintain confidentiality of patient information
- Compliance with the Acceptable Use Policy when using CareStar information systems
