About the role

Key responsibilities & impact
  • Provide information and assistance to link participants with medical, social and educational providers and other programs and services
  • Address identified needs and help participants achieve identified goals
  • Conduct health and safety, social and environmental assessments according to established guidelines
  • Serve on work groups, community groups, committees, or other task forces
  • Attend meetings as requested
  • Assess individual needs, develop personalized care plans, and connect individuals with essential services and supports

Requirements

What you’ll need
  • Bachelor's degree in human service field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing (or closely related field)
  • Two year's direct service experience serving persons with DD/IDD or related disabilities
  • Two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting
  • Experience in completing assessments, service plan development and monitoring
  • Effective organizational and communication skills
  • Experience providing care management/coordination preferred
  • Willingness and ability to work a flexible schedule to meet the needs of families
  • Reliable transportation
  • Valid driver's license and car insurance as required by state law
  • Ability to travel
  • Adherence to the CareStar Rule, CareStar Policies and Procedures, confidentiality requirements, and Acceptable Use Policy
