About the role

Key responsibilities & impact
  • Provide information and assistance linking participants with medical, social, and educational providers and other programs and services
  • Address identified needs and help participants achieve identified goals
  • Conduct health, safety, social, and environmental assessments according to established guidelines
  • Serve on work groups, community groups, committees, and task forces as requested
  • Attend meetings as requested
  • Assess individual needs, develop personalized care plans, and connect individuals with essential services and supports

Requirements

What you’ll need
  • Bachelor's degree in a human service field such as Social Work, Psychology, Sociology, Counseling, Gerontology, Nursing, or a closely related field
  • Two years' direct service experience serving persons with DD/IDD or related disabilities
  • Two years' experience in a healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department, or similar setting
  • Experience completing assessments, developing service plans, and monitoring services
  • Effective organizational and communication skills
  • Experience providing care management/coordination preferred
  • Willingness and ability to work a flexible schedule to meet family needs
  • Reliable transportation
  • Valid driver's license and car insurance as required by state law
  • Ability to travel
  • Adherence to the CareStar Rule and CareStar policies and procedures
  • Ability to maintain confidentiality of patient information
  • Compliance with the Acceptable Use Policy when using CareStar information systems
