About the role

Key responsibilities & impact
  • Provide social work services to patients and families to meet psychosocial needs, enhance problem-solving and coping capacities, and facilitate timely discharge
  • Arrange post-discharge services and assist with new Senior Nursing Facility (SNF) placement
  • Address acute social determinants and collaborate with healthcare team members on communication and care planning
  • Investigate uninsured or self-pay cases to identify insurance coverage and financial assistance needs
  • Evaluate patients for abuse or neglect and make appropriate referrals
  • Involve patients and families in case planning decisions
  • Keep the director or manager informed of problematic cases, especially legal or risk management issues
  • Assist and advocate for patients obtaining financial resources and government entitlements
  • Provide social work intervention, support, and planning for death, adoption, and fetal demise cases
  • Respond to healthcare team referrals by identifying facilities and services for during and after hospitalization
  • Help identify Health Care Surrogates under the Health Care Surrogacy Act
  • Assess uninsured or underinsured patients for Illinois Medicaid and Carle Financial Assistance Program eligibility
  • Complete PASRR assessments for discharge to extended care facilities
  • Complete mandated reports of suspected abuse or neglect and collaborate with investigating bodies on safe plans
  • Arrange transportation for medical appointments or discharge home when possible
  • Educate and connect patients with community resources, including in-home help, meals, housing, charity assistance, and support groups
  • Provide empathic listening and emotional support in person or by phone

Requirements

What you’ll need
  • Master's Degree in Social Work
  • Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR), or Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients
  • Ability to coordinate multiple projects concurrently and establish priorities
  • Strong planning and organizational skills
  • Self-motivated
  • Flexibility in hours and job duties
  • Ability to work effectively with diverse individuals and groups
