About the role
Key responsibilities & impact- Provide direct patient care through anticoagulation management under a collaborative practice agreement
- Implement, monitor, and manage anticoagulation therapy regimens
- Review AMS INRs and assign calls according to protocol
- Call and triage patients with abnormal INR results, assess clinical status, and determine next steps
- Manage warfarin therapy according to protocol or collaborative practice agreement
- Provide education on anticoagulation therapy, INR results, dosing instructions, and safety
- Implement and monitor peri-procedural bridging therapies
- Supervise AMS staff and pharmacy technicians
- Develop clinical guidelines and order sets related to drug therapy
- Intervene to improve prescribing, administration, and monitoring practices
- Represent the pharmacy on committees or work groups
- Screen medications for potential interactions
- Provide pre- and post-procedure guidance and assist with transitions of care
- Coordinate hospital discharges with the inpatient pharmacy team
- Train and orient new employees as assigned
- Provide Medication Therapy Management services
- Maintain regulatory compliance
Requirements
What you’ll need- Licensed Pharmacist - Illinois Department of Financial and Professional Regulation (IDFPR)
- Bachelor's Degree: Pharmacy
- Illinois pharmacist licensure
- Knowledge of anticoagulation management, warfarin, and direct oral anticoagulants
- Ability to implement, monitor, and adjust anticoagulation and peri-procedural bridging therapies
- Ability to provide Medication Therapy Management services as defined in the Illinois Pharmacy Practice Act
- Compliance with government and commercial regulations
- Ability to screen medications for interactions involving medications, diet, disease state, or allergies
- Ability to make dosing and therapy recommendations based on patient-specific factors
- Ability to supervise and instruct AMS pharmacy technicians
- Ability to provide patient education and manage patient communications
