About the role

Key responsibilities & impact
  • Complete documentation according to policy and procedure to maintain accurate clinical records
  • Report changes in patient status to licensed staff
  • Provide personal care and services according to the plan of care and orders
  • Assess, identify, and report safety issues
  • Assist with ambulation, exercise, walkers, canes, wheelchairs, and therapist-prescribed activities
  • Perform personal care, hygiene, treatments, and homemaking tasks
  • Assist with physician-ordered treatments such as colostomy bag changes, shampooing, simple wound care, dressing reinforcement, urine tests, and measuring intake and output
  • Encourage mental alertness, independence, and provide emotional and psychological support
  • Assist with bathing, personal care, hair care, skin care, and monitor vital signs
  • Participate in interdisciplinary team meetings and provide patient care updates

Requirements

What you’ll need
  • Certified nursing assistant
  • High school diploma/GED
  • Basic Life Support (BLS) within 30 days from the American Heart Association (AHA)
  • Illinois driver's license
  • Proof of auto insurance (varies)
  • Ability to perform home health and hospice patient care under the guidance and supervision of a registered nurse or therapist
