About the role
Key responsibilities & impact- Perform diagnostic assessments and collect diagnostic information
- Determine immediate treatment plan needs and request medical consultation when indicated
- Complete treatment plans with patients, identifying goals and objectives
- Provide behavioral health therapy to individuals, families, and groups
- Provide crisis evaluation and intervention as needed
- Coordinate with Emergency Response Service and other crisis service teams for clients who may be a danger to themselves or others
- Maintain records in accordance with UnityPlace policies and state, federal, Medicaid, accreditation, and insurance requirements
- Liaise between community resources and patient needs
- Coordinate treatment goals with community agencies, primary care physicians, courts, and managed-care entities
- Meet clinical productivity and documentation compliance standards
- May supervise UnityPlace clinicians and counseling or social work students
- Participate in in-service and competency training for students or staff
- Prepare special reports on activities and work-group activities
Requirements
What you’ll need- Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage & Family Therapist (LMFT), or Licensed Clinical Psychologist through the Illinois Department of Financial and Professional Regulation (IDFPR)
- Master's Degree in Social Work, Psychology, Counseling, or a related field
- 2 years of behavioral health experience
- Ability to use usual and customary equipment used to perform essential functions of the position
- Must meet clinical productivity and documentation compliance standards
- Work schedule: Monday–Friday, up to two evenings per week; variable shift
- Upon hire, work authorization is verified through E-Verify
