About the role
Key responsibilities & impact- Assist under RN direction in providing high-quality nursing care to home health patients
- Provide safe home health care services and follow the RN-established plan of care
- Conduct client assessments and complete visit documentation
- Implement nursing interventions according to client needs and changing conditions
- Communicate changes in client condition and client/caregiver needs to RNs and other staff
- Participate in client case reports and coordinate care with staff, physicians, and other resources
- Perform observations, test procedures, lab testing, and measures to prevent complications or minimize disease or injury effects
- Provide intermittent skilled care in accordance with the Plan of Treatment
- Maintain complete, legible, accurate, and timely clinical records, including daily visit notes, change orders, audit issues, occurrence reports, and follow-up communications
- Organize visit schedules and prioritize caseload needs to maximize client care
- Attend quarterly departmental meetings and assist with extra skilled nursing visits and weekend/holiday visits as requested
- Submit cellular phone statements/payments and payroll timesheets accurately and on time
- Pursue professional development and remain current with healthcare and departmental developments
- Educate clients, caregivers, and home health team members on health information
- Participate in departmental educational activities and work groups/teams
- Report to the assigned Home Health Clinical Manager
Requirements
What you’ll need- Licensed Practical Nurse (LPN) certification from the Illinois Department of Financial and Professional Regulation (IDFPR)
- Illinois driver's license from the Illinois Secretary of State (ILSOS)
- Proof of auto insurance
- Basic Life Support (BLS) certification from the American Heart Association within 30 days
- 1 year of nursing experience
- Must meet and maintain departmental and Carle Health educational requirements and required certifications
- Must obtain and maintain required technical skills, competencies, and certifications
- Knowledge of payer source requirements, professional practice standards, Carle Health policies and procedures, and regulatory requirements
- Ability to work Monday-Friday, 8:00 a.m.-4:30 p.m., every third weekend, two holidays per year, and on-call every third weekend and as needed
- Ability to travel to clients in the Peoria region
