About the role

Key responsibilities & impact

Ensure patient's safety by providing a safe environment
Directly observe identified patient(s) at all times during the assigned shift
Promote diversionary activities as necessary
Remove potentially dangerous items from patient rooms and surroundings
Ensure implementation of patient safety protocols, including elopement precautions
Maintain patient privacy and confidentiality
Assist patients with routine activities
Assist with meal selection and meal tray setup when appropriate and permitted
Answer patient phone
Notify nursing staff when assistance is needed for toileting, ambulation, or other activities
Coordinate care with appropriate hospital personnel
Consult with Lead RN, assigned RN, Security, and other staff as needed
Report oral intake and patient observations to the assigned RN
Report changes in patient behavior to nursing staff in a timely manner
Document implementation of safety protocols and proof of observation on flow sheets
Ensure safety protocols are utilized according to policy

Provide additional assistance to keep identified patients safe from self-harm
Direct observation of identified patients at all times during assigned shift
Ability to ensure patient safety and maintain a safe environment
Ability to promote diversionary activities as necessary
Ability to remove potentially dangerous items from patient rooms/surroundings
Ability to implement and document patient safety protocols, including elopement precautions
Ability to maintain patient privacy and confidentiality
Ability to assist with routine activities, meal selection, meal tray setup, toileting, and ambulation
Ability to coordinate care with hospital personnel, Lead RN, assigned RN, Security, and other staff
Ability to report oral intake, observations, and behavioral changes to assigned RN/nursing staff
Ability to document proof of observation and protocol implementation on flow sheets
