About the role

Key responsibilities & impact
  • Perform, coordinate, and supervise pharmacy services for home and alternative-site infusion therapies
  • Prepare and dispense drugs, supplies, equipment, and sterile products
  • Provide clinical assessments, education, care planning, and monitoring of therapy responses and outcomes
  • Communicate therapeutic drug monitoring results to physicians, nurses, and other appropriate personnel
  • Complete accurate clinical documentation and maintain pharmacy records
  • Assess patient, drug, and disease information to identify drug-related problems
  • Formulate goal-directed plans of care based on patient problems and outcomes
  • Intervene to improve medication prescribing, administration, and monitoring practices
  • Perform billing functions, patient charging, payer identification, and authorization acquisition
  • Program infusion control devices and teach patients, caregivers, and staff their use
  • Provide drug consultation to patients, families, physicians, and healthcare professionals
  • Supervise and participate in performance improvement activities
  • Supervise pharmacy operations and personnel
  • Assume manager/leader and Pharmacist-in-Charge duties when authorized and necessary

Requirements

What you’ll need
  • Licensed Pharmacist - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Bachelor's Degree in Pharmacy (Required)
  • Pharmacist work experience
  • Ability to prepare and dispense drugs, supplies, equipment, and sterile products
  • Knowledge of applicable federal, state, and local laws and standards of practice
  • Ability to use CareTend, NewLeaf, EPIC, TherigySTM, and Abacus
  • Ability to program infusion control devices and teach patients, caregivers, and staff how to use them
  • Full-time pharmacists are required to participate in an on-call rotation
