About the role
Key responsibilities & impact- Assist rounding nurses and providers by triaging calls for new SNFist Team admissions
- Serve as the SNFist Team point of contact and liaison among hospitalists, providers, rounding nurses, nursing homes, and related teams
- Perform telephone information intake and gather pertinent patient data for pending admissions
- Deliver nursing care efficiently using a standards-based framework
- Provide patient care recommendations within scope of practice and physician-approved protocols
- Complete telephone communications with patients, families, pharmacies, nursing homes, and healthcare facilities
- Provide written instructions regarding procedures, medications, and follow-up at the provider's direction
- Assist with forms and DME forms
- Facilitate nursing home admissions and continuity of care
- Address department and external operational issues to optimize patient care
- Communicate about and validate patient care
- Collaborate with SNFist Team nurses, PSR/MA, providers, nursing home DON and staff, hospitalist team, and case management
- Travel occasionally to long-term care facilities to foster teamwork and provide education
- Prepare pre-charting information, including medication reconciliation and pertinent medical information summaries
- Cover for rounding RNs as needed
Requirements
What you’ll need- Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
- Bachelor's Degree
- H.S. Diploma/GED
- Professional telephone etiquette and strong communication skills
- Ability to occasionally travel to long-term care facilities
- Ability to work Monday-Friday, 8:00 a.m.-4:30 p.m.
- Must participate in work authorization verification through E-Verify
