About the role

Key responsibilities & impact
  • Oversee, coordinate, and manage functional and financial outcomes during acute illness requiring hospitalization
  • Conduct proactive initial assessments, ongoing evaluations, and discharge planning
  • Facilitate safe and timely transitions from acute care to appropriate post-hospital care
  • Act as a liaison among patients, families, physicians, and post-hospital providers
  • Conduct case review presentations and educate peers on challenging cases and scope-of-practice issues
  • Coordinate transitions from inpatient to post-hospital care
  • Document plans of care and utilization issues in case management/utilization review software and multidisciplinary care documents
  • Evaluate plan-of-care effectiveness and initiate interventions before, during, and after hospitalization
  • Initiate referrals to healthcare team members and arrange services
  • Investigate post-hospital coverage and present options to patients, families, and providers
  • Lead interdisciplinary teams to achieve length-of-stay and readmission goals
  • Track avoidable inpatient days and conduct readmission assessments
  • Participate in daily multidisciplinary and white board rounds
  • Arrange DME, home care, hospice, ECF returns, transportation, and care conferences
  • Make follow-up appointments and post-discharge phone calls
  • Obtain prior authorizations, maintain work queues, address denials, and perform initial utilization review for emergency-room admissions

Requirements

What you’ll need
  • Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
  • Accredited Case Manager (ACM) within 3 years - American Case Management Association (ACMA)
  • College Diploma: Nursing
  • Weekend availability: Saturday/Sunday plus one weekday (Wednesday/Thursday/Friday)
  • Holiday availability if a holiday falls on the weekend
  • On-call availability for house from 4:30p-9p
