About the role
Key responsibilities & impact- Provide social work services to patients and families in the Emergency Department
- Meet identified psychosocial needs and enhance problem-solving and coping capacities
- Facilitate timely discharge by connecting patients and families with community resources
- Conduct psychosocial assessments
- Provide supportive counseling, trauma-informed care, crisis intervention, empathic listening, and emotional support
- Provide mobile mental and behavioral health services in the community with EMS
- Respond to traumas, victims of violence, and mental health patients in the ED
- Coordinate with security and spiritual care regarding next-of-kin notification
- Connect victims with community programs and outpatient social work
- Collaborate on cases involving SANE patients, deaths, medication assistance, abuse/neglect, chemical dependency, rehabilitation, advance directives, guardianship, and sex trafficking
- Attend leadership and community-resource meetings
- Evaluate abuse/neglect and assist with mandated reports and safe plans
- Involve patients and families in case-planning decisions
- Inform the director/manager about problematic cases, including legal or risk-management issues
- Counsel patients on advance directives, power of attorney, and conservatorship
- Respond to healthcare-team referrals and identify facilities and services
- Help patients obtain prescription medications through assistance programs
- Arrange transportation to medical appointments or home from the ED
- Educate and connect patients with resources for in-home help, food, housing, charity assistance, and support groups
- Collaborate with healthcare team members to ensure clear communication and care planning
Requirements
What you’ll need- Master's Degree in Social Work or Bachelor's Degree in Social Work
- Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR), or Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Ability to coordinate multiple projects concurrently and establish priorities
- Strong planning and organizational skills
- Self-motivated
- Flexibility in hours and job duties
- Ability to work effectively with diverse individuals and groups
- Ability to work night shifts: 2, 12-hour shifts per week, 7 p.m.–7 a.m.
- Ability to perform field response into the community, partnered with EMS
- Participation in department rotation for weekends and holidays
