About the role
Key responsibilities & impact- Identify psychosocial needs of patients and families through assessment
- Provide psychosocial assessments addressing emotional, social, and environmental strengths and problems
- Develop bereavement plans of care for hospice family members and caregivers
- Provide resource support, short-term counseling, and emergent crisis intervention
- Collaborate with interdisciplinary teams and develop social work plans of care
- Educate patients and families, including on advanced directives, living wills, HCPOA, and POLST forms
- Document assessments, financial assessments, interventions, visit notes, orders, and care-plan corrections in the electronic medical record
- Connect patients and families with community resources, transportation, food, clothing, utility, home-help, Medicaid, Community Care, and SSDI assistance
- Make elder abuse and neglect referrals
- Evaluate and modify social work interventions as indicated
- Support patient decisions, cultural beliefs, values, and end-of-life care needs
- Address patient and family needs at time of death
- Apply knowledge of Medicare Home Health and Hospice benefits
- Utilize Memorial Funds appropriately and submit required documentation
Requirements
What you’ll need- Master’s Degree in Social Work
- Licensed Social Worker (LSW) issued by the Illinois Department of Financial and Professional Regulation (IDFPR), or Licensed Clinical Social Worker (LCSW) issued by IDFPR
- Illinois driver’s license
- Proof of auto insurance
- 1+ years of social worker experience in a healthcare setting
- 1 year of experience post social work license
- Ability to work as part of a team
- Previous Hospice experience preferred
