About the role
Key responsibilities & impact- Implement the Family Connections model and curriculum with fidelity and follow program and agency policies and procedures
- Complete Family Connections core training and DCS Standards of Practice
- Conduct home visits and maintain an assigned caseload
- Complete individual home visits at least once a week and deliver the Family Connections curriculum
- Prepare for home visits and document each visit within one business day
- Conduct visits at times convenient for families, including evenings and weekends
- Conduct comprehensive family and other required assessments
- Work with DCS Specialists to engage and assess families
- Develop and implement Service Plans with families, including UBSMART goals and supporting interventions
- Maintain case records and data systems
- Contact referred families and conduct an initial home visit within one business day of referral
- Attend required virtual or in-person trainings, conferences, meetings, and community events, including possible out-of-state travel
- Connect families with resources and make referrals to government agencies, community agencies, and program subcontractors
- Follow up with subcontractors regarding progress
- Report suspected child abuse or neglect to the Department of Child Safety
- Complete training in and utilize the Nurturing Parenting Program model
- Conduct intakes and complete Family Nurturing Plans with parents/caregivers
- Conduct at least two hours of weekly in-person parenting sessions and document family interaction and progress
- Schedule and conduct midpoint sessions to evaluate progress toward Nurturing Parenting goals
- Complete case closures and required closure documentation
- Participate in weekly individual and group supervision
- Maintain positive working relationships with colleagues, administration, collaborative partners, and community members
- Promote quality improvement and perform other duties as assigned
Requirements
What you’ll need- Minimum of a Bachelor’s Degree in Social Work or related field
- At least one (1) year of relevant experience
- Knowledge of community resources, case management, assessment and service planning, and problem solving with complex family needs
- Understanding of DCS systems preferred
- Ability to work under pressure, meet deadlines, set priorities, manage multiple cases, and work independently and as part of a team
- Flexibility and ability to adapt to changing working conditions
- Sensitivity to cultural differences within the agency and community
- Fluency in Microsoft Office Applications
- Intermediate-level technology and software use for documentation and case work
- Ability to stoop, bend, squat, sit on floor, and ambulate over uneven floor surfaces including stairs and steps
- Ability to visually and auditorily assess and interact with clients to ensure safety and wellbeing
- Willingness to work in off-site home environments
- Willingness to be on-call for emergencies
- Availability for evenings and weekends
- Ability to lift up to 25 pounds
- Valid AZ driver’s license, reliable transportation, and proof of registration/insurance
- Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance
- Must be 21 years of age or older to drive on behalf of Catholic Charities
- Must have or complete within 90 days of hire training in CPR and First Aid and remain current in the certification
