About the role
Key responsibilities & impact- Support identifying, overseeing, and managing coordination of member transitions in the community
- Work with leadership to ensure timely and safe transitions across levels of health care services
- Coordinate care plans with community care coordinators
- Educate transition enrollees about services, requirements, limitations, and exclusions
- Perform or assist with member assessments and screenings
- Develop or assist with member transition, service, or care plans
- Support care coordination teams, providers, and health care team members in developing effective transition plans
- Assist with member transitions based on enrollment or transition of care
- Identify new enrollees requiring transition services
- Ensure existing authorizations are honored during transitions and address issues with care management teams and providers
- Serve as a resource for members, families, and caregivers
- Track and maintain transition metrics and identify trends and process improvements
- Document transition-of-care information
- Draft education materials and resources
- Assist with education and training programs for care coordination staff and providers
- Evaluate member needs and available resources and recommend and facilitate plans
- Coordinate among members, families, caregivers, and care provider teams
- Interact with health care providers to facilitate care coordination
- Perform other duties as assigned
Requirements
What you’ll need- Must be authorized to work in the U.S. without employment-based visa sponsorship now or in the future
- Bachelor's degree
- 2–4 years of related experience or equivalent experience acquired through applicable knowledge, duties, scope and skill
- Preferred behavioral health experience
- Preferred patient- or member-facing experience
- Strong verbal communication skills
- Ability to effectively engage with patients, members, and providers by phone
- Prefer candidates located within Texas
- Ability to comply with all policies and standards
