About the role

Key responsibilities & impact
  • Support care management activities and assigned member teams
  • Ensure services are delivered by healthcare providers and partners
  • Perform member outreach by telephone and home visits
  • Document care/service plans, activities, member needs, and records
  • Discuss next steps, resources, questions, concerns, recommended care, and ongoing education with members
  • Coordinate care activities with healthcare providers, community providers, partners, members, and caregivers
  • Respond to member and provider inquiries, requests, and concerns
  • Communicate with care managers, practitioners, and others to facilitate services and continuity of care
  • Support service assessments and screenings
  • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
  • Provide education on available benefits and resources
  • Follow standards of practice and comply with contractual, regulatory, and organizational requirements
  • Perform other duties as assigned

Requirements

What you’ll need
  • High School diploma or GED
  • 1–2 years of related experience
  • Bachelor's degree in social science/health-related field OR high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
  • Must reside in Washington, Benton, or Madison County, Arkansas
  • Must reside in Arkansas or a border city
  • Driver's license required
  • Child and adult maltreatment check required before hire and recurring
  • Drug screen required at time of hire and recurring
  • Ability to travel; travel is 30%
  • Ability to support members with developmental disabilities and behavioral health needs
  • Knowledge of care plans/service plans, member outreach, care coordination, referrals, benefits, and community resources
  • Compliance with state, contractual, regulatory, and organizational policies and standards
