About the role
Key responsibilities & impact- Support care management activities and assigned member teams
- Ensure services are delivered by healthcare providers and partners
- Perform member outreach by telephone or home visits
- Discuss care plans/service plans, next steps, resources, questions, concerns, and ongoing education
- Coordinate care activities with healthcare and community providers, partners, members, and caregivers
- Support member and provider inquiries, requests, and concerns related to care plans/service plans
- Communicate with care managers, practitioners, and others to facilitate member services and continuity of care
- Support service assessments/screenings and document member care needs
- Document and maintain member records according to state and regulatory requirements
- Distribute records to providers as needed
- Identify needs and make referrals to care managers, community-based organizations, and disease managers
- Provide education on available benefits and resources
- Perform other assigned duties and comply with all policies and standards
Requirements
What you’ll need- Must reside in Kansas
- Behavioral Health experience preferred
- High School diploma or GED
- 1–2 years of related experience
- For Arkansas Total Care plan: Bachelor's degree in social science/health-related field OR high school diploma with at least one (1) year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
- For Arkansas Total Care plan: driver's license required
- For Arkansas Total Care plan: child and adult maltreatment check required before hire and recurring
- For Arkansas Total Care plan: drug screen required at time of hire and recurring
- For Arkansas Total Care plan: must reside in Arkansas or a border city
- For Arkansas Total Care plan: 30% travel required
- Compliance with policies, contractual requirements, and regulatory guidelines and standards
- Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager
