About the role

Key responsibilities & impact
  • Provide non-clinical administrative support to the care management team
  • Handle inbound and outbound calls supporting care management services
  • Manage call queues, tasks, and referrals
  • Gather information by phone to determine eligibility for care management enrollment
  • Route referrals to appropriate clinical or nursing teams
  • Conduct member outreach by phone or home visits
  • Discuss care plans/service plans, next steps, resources, questions, and concerns with members
  • Coordinate care activities with healthcare and community providers, partners, members, and caregivers
  • Support member and provider inquiries related to care plans/service plans
  • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care
  • Support service assessments/screenings and document member care needs
  • Document and maintain member records and distribute them to providers as needed
  • Provide education on benefits and resources
  • Perform other duties as assigned and comply with policies and standards

Requirements

What you’ll need
  • High School diploma or GED
  • 1–2 years of related experience
  • Ability to handle inbound and outbound calls
  • Ability to manage call queues, tasks, and referrals in a timely manner
  • Ability to gather information by phone to determine eligibility for care management enrollment
  • Ability to route referrals to appropriate clinical or nursing teams
  • Ability to perform member outreach telephonically or through home visits
  • Ability to document and maintain member records in accordance with state and regulatory requirements
  • Ability to follow contractual requirements, regulatory guidelines, and standards
  • Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager
  • Ability to provide education on benefits and available resources
