About the role
Key responsibilities & impact- Support care management activities and assigned member teams
- Ensure services are delivered by healthcare providers and partners
- Conduct member outreach by telephone or home visits
- Document care/service plans and related activities
- Coordinate care activities with healthcare providers, community partners, members, and caregivers
- Respond to member and provider inquiries, requests, and concerns
- Communicate with care managers, practitioners, and others to facilitate member services and continuity of care
- Support service assessments and screenings
- Maintain member records in accordance with state and regulatory requirements
- Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
- Educate members about available benefits and resources
- Perform other assigned duties and comply with policies and standards
Requirements
What you’ll need- High School diploma or GED
- 1–2 years of related experience
- For Arkansas Total Care plan: Bachelor's degree in social science/health-related field OR a high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
- Must reside within Phillips, Monroe, or Lee County, Arkansas; or in Arkansas or a border city
- Driver's license required
- Child and adult maltreatment check required before hire and recurring
- Drug screen required at time of hire and recurring
- Ability to travel; travel is 30%
- Position designated as safety sensitive in Arkansas
