About the role
Key responsibilities & impact- Support care management activities and assigned member teams to ensure provider and partner services are delivered and continuity of care/member satisfaction is achieved
- Conduct member outreach telephonically or through home visits
- Document care/service plans, activities, member needs, and ongoing education
- Engage members to discuss care plans, next steps, resources, questions, concerns, and recommended care
- Coordinate care activities with healthcare and community providers, partners, members, and caregivers
- Support member and provider inquiries, requests, and concerns related to care/service plans
- Communicate with care managers, practitioners, and others to facilitate member services and continuity of care
- Support service assessments and screenings for members
- Maintain member records in accordance with state and regulatory requirements and distribute them to providers as needed
- Provide education on benefits and available resources
- Perform other assigned duties and comply with policies and standards
Requirements
What you’ll need- High School diploma or GED
- 1–2 years of related experience
- For Arkansas Total Care plan: Bachelor's degree in social science/health-related field, or a high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
- For Arkansas: driver's license
- For Arkansas: child and adult maltreatment check before hire and recurring
- For Arkansas: drug screen at time of hire and recurring
- For Arkansas: must reside in Arkansas or a border city
- For Arkansas: 30% travel required
- Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager
- Ability to provide education on benefits and available resources
- Compliance with contractual requirements, regulatory guidelines, standards of practice, and policies
