About the role

Key responsibilities & impact
  • Support care management activities and assigned member teams
  • Perform member outreach by telephone and home visits
  • Document care/service plans and related activities
  • Coordinate care with healthcare providers, community providers, partners, members, and caregivers
  • Respond to member and provider inquiries, requests, and concerns
  • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care
  • Support service assessments and screenings and document member care needs
  • Maintain member records according to state and regulatory requirements and distribute them to providers as needed
  • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
  • Educate members about benefits and available resources
  • Perform other assigned duties and comply with all policies and standards

Requirements

What you’ll need
  • High School diploma or GED
  • 1–2 years of related experience
  • For Arkansas Total Care plan: Bachelor's degree in social science/health-related field OR a high school diploma with at least one (1) year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
  • Must reside in Arkansas or a border city; for this field-based role, must reside within one of the assigned counties in Arkansas
  • Driver's license required
  • Child and adult maltreatment check required before hire and recurring
  • Drug screen required at time of hire and recurring
  • Ability to travel 30%
  • Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager
  • Knowledge of benefits and available resources sufficient to provide member education
  • Ability to comply with contractual requirements, regulatory guidelines, policies, and standards
