About the role

Key responsibilities & impact
  • Support care management activities and assigned member-care teams
  • Ensure services are delivered by healthcare providers and partners
  • Promote continuity of care and member satisfaction
  • Conduct member outreach by telephone or home visits
  • Document care and service activity plans
  • Discuss care plans, next steps, resources, questions, concerns, and ongoing education with members
  • Coordinate care activities with healthcare and community providers, partners, members, and caregivers
  • Respond to member and provider inquiries, requests, and concerns
  • Communicate with care managers, practitioners, and others to facilitate member services
  • Support service assessments and screenings
  • Document and maintain member records and distribute them to providers as needed
  • Follow contractual, regulatory, and organizational standards and policies
  • Provide education on available benefits and resources
  • Perform other duties as assigned

Requirements

What you’ll need
  • High School diploma or GED
  • 1–2 years of related experience
  • Previous experience with the Foster Care population is strongly preferred
  • Ability to work Monday–Friday, 8am–5pm
  • Ability to work remotely from home anywhere within the state of Missouri
  • Ability to provide telephonic support to Foster Care children
  • Ability to perform member outreach by phone or home visits
  • Ability to coordinate care with healthcare and community providers, partners, members, and caregivers
  • Ability to identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
  • Ability to document and maintain member records in accordance with state and regulatory requirements
