About the role

Key responsibilities & impact
  • Support the senior care management team and teams assigned to members
  • Ensure healthcare providers and partners deliver services and continuity of care/member satisfaction is achieved
  • Serve as a liaison with care managers and providers to coordinate member care
  • Conduct member outreach by phone and home visits
  • Discuss care plans/service plans, next steps, resources, questions, concerns, and ongoing education with members
  • Coordinate care activities with healthcare and community providers, partners, members, and caregivers
  • Respond to member and provider inquiries, requests, and concerns
  • Develop knowledge of care management services and respond to complex or escalated issues
  • Communicate with care managers, practitioners, and others to facilitate member services
  • Perform service assessments/screening for members with complex needs and document care needs
  • Maintain member records in accordance with state and regulatory requirements and distribute records to providers as needed
  • Triage, adjust, and escalate complex requests to management
  • Follow contractual, regulatory, and organizational standards and policies
  • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
  • Provide education on benefits and available resources
  • Assist with training and development needs as needed
  • Perform other duties as assigned

Requirements

What you’ll need
  • Applicants must reside in Illinois, specifically one of the listed southern IL counties: Alexander, Bond, Clay, Clinton, Crawford, Edwards, Effingham, Fayette, Franklin, Gallatin, Hamilton, Hardin, Jackson, Jasper, Jefferson, Johnson, Lawrence, Madison, Marion, Massac, Monroe, Perry, Pope, Pulaski, Randolph, Richland, Saline, St. Clair, Union, Wabash, Washington, Wayne, Wayne City, White, Williamson
  • Bachelor's degree in nursing, social sciences, social work, or related human services/social services field for the Illinois YouthCare plan
  • One (1) year of supervised clinical experience in a human-services field for the Illinois YouthCare plan
  • High School diploma or GED
  • 2–4 years of related experience
  • Must be available Monday–Friday, 8am–5pm Central Time
  • Ability to perform member outreach by phone and home visits
  • Knowledge of care management services
  • Ability to perform service assessments/screening and document member care needs
  • Ability to document and maintain member records according to state and regulatory requirements
  • Ability to triage, adjust, and escalate complex requests
  • Ability to identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
