About the role
Key responsibilities & impact- Develop, assess, and facilitate complex care management activities for members with primarily mental and behavioral health needs
- Create personalized care plans and educate members and families about mental health and substance use disorder needs
- Evaluate member needs by phone or in-home visits and recommend or facilitate care and service plans
- Conduct telephonic, digital, home, and other site-visit outreach
- Develop ongoing care plans for members with high acuity
- Identify providers, specialists, and community resources for mental health and substance use disorders
- Coordinate among members, families, caregivers, community resources, and care provider teams
- Monitor care plans, member status, treatment side effects, complications, and clinical symptoms
- Facilitate care coordination and timely access to needed services
- Document member information and care management activities
- Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits
- Provide leadership feedback on opportunities to improve quality and cost-effective care delivery
- Perform other duties as assigned
- Comply with all policies and standards
Requirements
What you’ll need- Applicants must reside in Kansas
- Active Licensed Master’s Behavioral Health Professional license (e.g., LCSW, LMSW, LMFT, LMHC, or LPC) or RN requirements with behavioral health experience
- Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
- 2–4 years of related experience
- Behavioral health clinical knowledge and assessment skills
- Experience with discharge planning, problem-solving, care coordination, computer systems, organizational management, time management, and multidisciplinary teams highly preferred
- Ability to work Tuesday through Saturday, 10:00 AM to 6:00 PM, or Sunday through Thursday, 10:00 AM to 6:00 PM
- Must comply with applicable policies and standards
- Must collect, document, and maintain member information in compliance with state, federal, and third-party payer regulators
