About the role
Key responsibilities & impact- Develop, assess, and facilitate complex care management activities for members with primarily mental and behavioral health needs
- Provide personalized care plans and education for members and families related to mental health and substance use disorder
- Evaluate member needs by phone or in-home visits and recommend or facilitate care/service plans
- Conduct telephonic, digital, home, and other site-visit outreach
- Develop ongoing care plans for high-acuity members and identify providers, specialists, and community resources
- Coordinate among members, families/caregivers, community resources, and care provider teams
- Monitor care plans, member status, outcomes, treatment side effects, complications, and clinical symptoms
- Facilitate care coordination and collaborate with providers and specialists to ensure timely access to care
- Collect, document, and maintain member information and care management activities for regulatory compliance
- Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits
- Provide leadership feedback on opportunities to improve cost-effective care and quality delivery
- Comply with all policies and standards and perform other duties as assigned
Requirements
What you’ll need- Applicants must reside in Kansas
- Must hold an active Licensed Master’s Behavioral Health Professional license (e.g., LCSW, LMSW, LMFT, LMHC, or LPC) or meet RN requirements with behavioral health experience
- Master's degree in Behavioral Health or Social Work, or a degree from an Accredited School of Nursing
- 2–4 years of related experience
- Behavioral health experience required for RN candidates based on state contract requirements
- Strong behavioral health clinical knowledge and assessment skills
- Excellent communication and customer service skills
- Experience with discharge planning, problem-solving, care coordination, computer systems, organizational management, time management, and multidisciplinary teams highly preferred
- Monday through Friday, 8:00 AM to 5:00 PM work schedule
