About the role

Key responsibilities & impact
  • Develop, assess, and facilitate complex care management activities for primarily physical-needs members
  • Evaluate member needs, care-access barriers, and social determinants of health
  • Develop ongoing care plans and service plans
  • Collaborate with providers, specialists, caregivers, and community resources
  • Identify problems and barriers to care and provide care management interventions
  • Coordinate person-centered care and services between members, families, caregivers, and provider teams
  • Monitor member status, condition changes, and progress toward care plan goals
  • Provide local resource support for services including employment, housing, participant direction, independent living, justice, and foster care
  • Facilitate timely access to needed care and services
  • Conduct telephonic, digital, home, and other site outreach
  • Collect, document, and maintain member information and care management activities for regulatory compliance
  • Educate members and families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits
  • Provide leadership feedback on opportunities to improve care quality and cost effectiveness
  • Perform other assigned duties and comply with all policies and standards

Requirements

What you’ll need
  • Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
  • 2–4 years of related experience
  • Active New Jersey RN license required
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required
  • Strong computer skills
  • Strong time-management skills
  • Strong organizational skills
  • Comfortable managing an MLTSS caseload in accordance with New Jersey Medicaid requirements
  • Approximately 50% travel for member visits
  • Monday–Friday availability from 8:00 a.m. to 5:00 p.m., with flexibility based on business needs
