About the role
Key responsibilities & impact- Develop, assess, and coordinate care management activities based on member needs
- Develop or contribute to personalized care plans/service plans
- Educate members and families/caregivers on services and benefit options
- Evaluate member needs, barriers to care, available resources, and recommend or facilitate plans
- Identify providers, specialists, and community resources needed for care
- Provide psychosocial and resource support for employment, education, housing, food, participant direction, independent living, justice, and foster care needs
- Coordinate members/families/caregivers with care provider teams to ensure timely access to care and services
- Monitor progress toward care plan goals and member status or changes in condition
- Collaborate with healthcare providers on care plan revisions and identified member needs
- Collect, document, and maintain member information and care management activities for regulatory compliance
- Perform on-site visits to assess member needs and collaborate with providers or resources
- Provide education on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits
- Perform other duties as assigned and comply with all policies and standards
Requirements
What you’ll need- Bachelor’s degree and 2–4 years of related experience
- Alternatively, equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position
- If holding clinical licensure, graduation from an Accredited School of Nursing
- Current state clinical license preferred
- Excellent communication and customer service skills
- Experience working with multidisciplinary teams
- Strong problem-solving abilities
- Computer proficiency
- Effective organizational and time-management skills
- Ability to work independently as a self-starter
- Must reside in Montgomery County, Ohio, or Butler, Warren, Greene, Clinton, Clark, Miami, or Darke County
- Ability to work Monday through Friday, 8:00 AM to 5:00 PM
