About the role

Key responsibilities & impact
  • Develop, assess, and coordinate care management activities based on member needs
  • Develop or contribute to personalized care plans/service plans
  • Educate members and families/caregivers on services, benefits, care options, referrals, and provider instructions
  • Evaluate member needs, barriers to care, available resources, and recommended outcomes
  • Identify providers, specialists, and community resources needed for care
  • Provide psychosocial and resource support related to employment, education, housing, food, participant direction, independent living, justice, and foster care
  • Coordinate between members/families/caregivers and care provider teams to ensure timely access to care and services
  • Monitor progress toward care plan goals and member status or condition changes
  • Collaborate with healthcare providers to revise care plans and address member needs
  • Collect, document, and maintain member information and care management activities for regulatory compliance
  • Perform on-site visits when appropriate
  • Perform other duties as assigned and comply with all policies and standards

Requirements

What you’ll need
  • Bachelor’s degree
  • 2–4 years of related experience
  • Graduate from an Accredited School of Nursing if holding clinical licensure
  • Current state’s clinical license preferred
  • Ability to work remotely from home anywhere within the state of Missouri
  • Ability to work Monday–Friday, 8am–5pm
  • Previous foster care experience strongly preferred
