About the role

Key responsibilities & impact
  • Develop, assess and coordinate care management activities based on member needs
  • Develop or contribute to personalized care plans and service plans
  • Educate members and families/caregivers about services, benefits and care options
  • Evaluate member needs, barriers to care and available resources
  • Identify and facilitate access to providers, specialists and community resources
  • Provide psychosocial and resource support for employment, education, housing, food, independent living, justice and foster care needs
  • Coordinate members/families/caregivers with care provider teams
  • Monitor care plan goals and member status, and collaborate on revisions or further evaluation
  • Collect, document and maintain member and care management information for regulatory compliance
  • Perform on-site visits when appropriate
  • Educate care managers and members/caregivers on procedures, provider instructions, referrals and healthcare benefits
  • Perform other assigned duties and comply with policies and standards

Requirements

What you’ll need
  • Bachelor’s degree and 2–4 years of related experience
  • Alternatively, equivalent experience acquired through applicable knowledge, duties, scope and skill reflective of the position level
  • If holding clinical licensure, graduation from an accredited school of nursing
  • Current state clinical license preferred
  • Ability to work Monday–Friday, 8am–5pm
  • Previous foster care experience strongly preferred
  • Ability to work remotely from home anywhere within Missouri
  • Telephonic outreach to foster care members/guardians
  • Knowledge of care management, healthcare access, care coordination, member assessments, personalized care plans and community resources
