About the role
Key responsibilities & impact- Develop, assess, and coordinate care management activities based on member needs
- Develop or contribute to personalized care plans/service plans
- Educate members and families/caregivers on services, benefits, procedures, care options, referrals, and provider instructions
- Evaluate member needs, barriers to care, available resources, and recommend or facilitate appropriate outcomes
- Identify providers, specialists, and community resources needed for care
- Provide psychosocial and resource support related to employment, education, housing, food, participant direction, independent living, justice, and foster care
- Coordinate members/families/caregivers with care provider teams to ensure timely access to services
- Monitor progress toward care plan goals and member status or changes in condition
- Collaborate with healthcare providers on care plan revisions and refer members for further care management evaluation
- Collect, document, and maintain member information and care management activities for regulatory compliance
- Perform on-site visits when appropriate to assess needs and collaborate with providers or resources
- Perform other duties as assigned and comply with company policies and standards
Requirements
What you’ll need- Bachelor’s degree and 2–4 years of related experience
- Graduate from an Accredited School of Nursing if holding clinical licensure
- Equivalent experience acquired through applicable knowledge, duties, scope and skill reflective of the position level may qualify
- Current state’s clinical license preferred
- Ability to work Monday–Friday, 8am–5pm
- Previous foster care experience strongly preferred
- Applicants must work remotely from their home anywhere within the state of Missouri
