About the role
Key responsibilities & impact- Develop, assess, and coordinate care management activities based on member needs
- Develop or contribute to personalized care plans/service plans
- Educate members, families, caregivers, care managers, DCFS staff, and stakeholders on services, benefits, care options, procedures, and healthcare instructions
- Evaluate member needs, barriers to care, and available resources
- Identify providers, specialists, and community resources
- Provide psychosocial and resource support for employment, education, housing, food, participant direction, independent living, justice, and foster care needs
- Coordinate members, families/caregivers, care providers, and healthcare teams to ensure timely access to care
- Monitor care plan goals and member status, collaborate on revisions, and refer for further evaluation
- Collect, document, and maintain member information and care management activities for regulatory compliance
- Conduct on-site visits and collaborate with providers and resources
- Provide administrative coordination with DCFS staff and stakeholders
- Engage in immediate problem resolution and communicate resolutions to appropriate staff and stakeholders
- Perform other assigned duties and comply with company policies and standards
Requirements
What you’ll need- Bachelor's degree in Social Work, Nursing, Health Behavioral Science or equivalent experience
- 2+ years of experience working within a social service agency and/or advocacy environment
- Must reside in Illinois
- Availability during regular work hours to communicate with and provide education and training to DCFS staff and stakeholders
- Monday-Friday training schedule, 8-5 pm
- Main schedule Monday-Friday 8-4:30 pm or 8:30-5 pm CT, with some weekend events
- Ability to perform approximately 25% local travel for DCFS and CWCA office visits in northern Cook County
- Ability to perform on-site visits as needed
