About the role

Key responsibilities & impact
  • Develops, assesses, and coordinates holistic care management activities for long-term care members
  • Develops or assists with personalized service care plans and educates members and caregivers on available services and benefits
  • Evaluates member needs and available resources and recommends or facilitates appropriate care plans
  • Identifies providers, specialists, and community resources for long-term care
  • Coordinates members, caregivers, and care provider teams to ensure services are accessible
  • Provides resource support for employment, housing, participant direction, independent living, justice, and foster care needs
  • Monitors care plans, member status, and outcomes and recommends updates based on member needs
  • Interacts with long-term care providers and partners
  • Collects, documents, and maintains member information and care management activities for regulatory compliance
  • May conduct on-site visits to assess member needs and collaborate with providers or resources
  • Educates members and caregivers on procedures, provider instructions, service options, referrals, and healthcare benefits
  • Provides leadership feedback on opportunities to improve quality of care and cost-effective service delivery
  • Performs other assigned duties

Requirements

What you’ll need
  • Bachelor's degree required and 1+ years of experience with populations served for the Iowa plan
  • Alternatively, RN with 6+ years of experience with population served for the Iowa plan
  • Requires a Bachelor's degree and 1 year of related experience, or equivalent experience acquired through applicable knowledge, duties, scope and skill
  • For Physically Disabled/Elderly populations, must meet one of: RN licensed in Illinois; bachelor's or master's degree in a human services-related field; or LPN with one year of experience conducting comprehensive assessments and providing formal services for the elderly
  • For Brain Injury/HIV/AIDS populations, must meet one of: Illinois-licensed RN with specified bachelor's degree or four years of case management experience; certified or licensed social worker with specified bachelor's or master's degree; or unlicensed social worker with a bachelor's degree in social work, social sciences, or counseling
  • Brain Injury/HIV/AIDS experience with addictive and dysfunctional family systems, racial and ethnic minorities, homosexuals and bisexuals, persons with AIDS, and substance abusers
  • For Superior Health Plan, direct experience working with individuals with disabilities and/or vulnerable populations with chronic or complex conditions, including children and young adults, within three of the last five years
  • Compliance with applicable state-specific requirements
  • Must comply with all policies and standards
