About the role
Key responsibilities & impact- Assist in developing, assessing, and coordinating holistic care management activities
- Develop or assist with personalized service care plans for long-term care members
- Educate members and families/caregivers on available services and benefits
- Evaluate member needs and available resources and recommend or facilitate care plans
- Identify providers, specialists, and community resources for long-term care
- Coordinate between members, caregivers, and care provider teams to ensure services are accessible
- Provide resource support for employment, housing, participant direction, independent living, justice, and foster care needs
- Monitor care plans, member status, and outcomes and recommend updates
- Interact with long-term care healthcare providers and partners
- Collect, document, and maintain member information and care management activities for regulatory compliance
- Conduct on-site visits and collaborate with providers or resources
- Provide education on procedures, healthcare provider instructions, service options, referrals, and healthcare benefits
- Provide leadership feedback on improving quality of care and cost-effective service delivery
- Perform other assigned duties and comply with policies and standards
Requirements
What you’ll need- Bachelor's degree and 1 year of related experience
- Equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position may qualify
- For Iowa Plan Only: Bachelor's degree required and 1+ years of experience with populations served, or RN with 6+ years of experience with population served
- Ability to conduct in-person member visits throughout Woodbury County, Iowa
