About the role

Key responsibilities & impact
  • Develop, assess, and coordinate holistic care management activities
  • Develop or assist with personalized service care plans for long-term care members
  • Educate members and families/caregivers on available services and benefits
  • Evaluate member needs and available resources and facilitate care plans
  • Identify providers, specialists, and community resources for long-term care
  • Coordinate members, caregivers, and care provider teams to ensure services are accessible
  • Provide resource support for employment, housing, participant direction, independent living, justice, and foster care needs
  • Monitor care plans, member status, and outcomes and recommend updates
  • Interact with long-term care providers and partners
  • Collect, document, and maintain member information and care management activities for regulatory compliance
  • Perform on-site visits and collaborate with providers or resources
  • Provide education on procedures, healthcare instructions, service options, referrals, and benefits
  • Provide leadership feedback to improve quality of care and cost-effective service delivery
  • Perform other assigned duties

Requirements

What you’ll need
  • Bachelor's degree and 1 year of related experience
  • For Illinois Plan Physically Disabled/Elderly services, one of the following: RN licensed in Illinois; bachelor's or master's degree in a human services related field; or LPN with one year of experience conducting comprehensive assessments and providing formal service for the elderly
  • Ability to work the specified training and main schedules
  • Compliance with all policies and standards
