About the role
Key responsibilities & impact- Develops, assesses, and coordinates holistic care management activities
- Develops or assists with personalized service care plans for long-term care members
- Evaluates member needs and available resources and recommends or facilitates care plans
- Identifies providers, specialists, and community resources for long-term care
- Coordinates members, families/caregivers, care providers, and partners to ensure services are accessible
- Provides resource support for employment, housing, participant direction, independent living, justice, and foster care needs
- Monitors care plans, member status, and outcomes and recommends updates
- Interacts with long-term care healthcare providers and partners
- Documents member information and care management activities for regulatory compliance
- May conduct home or other site visits
- Educates members and families/caregivers about procedures, provider instructions, service options, referrals, and healthcare benefits
- Provides leadership feedback on improving quality of care and cost-effective service delivery
- Performs other assigned duties and complies with policies and standards
Requirements
What you’ll need- Must reside in Orange or Osceola County, Florida
- Bachelor's degree and 2–4 years of related experience; or Bachelor's degree in health, human, social work or education services with one or more years of qualifying experience; or high school degree/equivalent and three years of qualifying case management experience
- Experience interviewing and assessing member needs
- Knowledge and experience regarding caseload management and casework practices
- Knowledge regarding determining eligibility for DHSS programs
- Knowledge of Federal and State law as it applies to DHSS programs
- Ability to solve problems and locate community resources
- Ability to collaborate with caregivers, State agency representatives and providers
- Good interpersonal skills
- Ability to practice cultural competency with awareness and respect for diversity
- Knowledge of the needs and service delivery system for populations in the case manager's caseload
- Computer savvy
- Bilingual Spanish skills preferred
