About the role
Key responsibilities & impact- Assist in developing, assessing, and coordinating holistic care management activities
- Develop or assist with personalized service care plans for long-term care members
- Educate members and families/caregivers on available services and benefits
- Evaluate member needs and available resources and facilitate plans for optimal outcomes
- Identify providers, specialists, and community resources for long-term care
- Coordinate between members, families/caregivers, and care provider teams
- Provide resource support for employment, housing, participant direction, independent living, justice, and foster care needs
- Monitor care plans, member status, and outcomes and recommend updates
- Interact with long-term care providers and partners
- Collect, document, and maintain member information and care management activities for regulatory compliance
- Conduct home and other site visits to assess needs and collaborate with providers and partners
- Provide education on procedures, provider instructions, service options, referrals, and healthcare benefits
- Provide leadership feedback on quality-of-care and service-delivery improvements
- Perform other duties as assigned
Requirements
What you’ll need- Bachelor's degree and 2–4 years of related experience
- For Iowa: Bachelor's degree and 2+ years of experience with populations served, or RN with 6+ years of experience with population served
- Experience interviewing and assessing member needs
- Knowledge and experience regarding caseload management and casework practices
- Knowledge regarding determining eligibility for DHSS programs
- Knowledge of Federal and State law as it applies to DHSS programs
- Ability to effectively solve problems and locate community resources
- Ability to collaborate with caregivers, involved State agency representatives and providers
- Good interpersonal skills
- Ability to practice Cultural Competency with awareness and respect for diversity
- Knowledge of the needs and service delivery system for all populations in the case manager’s caseload
- Equivalent experience acquired through applicable knowledge, duties, scope and skill may be considered
