About the role

Key responsibilities & impact
  • Develop, assess, and coordinate holistic care management activities for members with significant mental/behavioral health needs
  • Evaluate member service needs and develop or contribute to care plans and service plans
  • Educate members, families, and caregivers on available services and benefits
  • Evaluate complex and high-risk members and recommend plans of care
  • Act as liaison and member advocate between members/families, physicians, and facilities/agencies
  • Perform frequent home and other site visits, once a month or more, to assess needs and collaborate with resources
  • Provide or facilitate education on preventive care, procedures, provider instructions, treatment options, referrals, medications, and healthcare benefits
  • Provide subject matter expertise and operational support for behavioral health activities, including crisis calls, mental health first aid training, field safety, de-escalation, and medication monitoring
  • Educate on and coordinate medical, behavioral, and social community resources
  • Coordinate service authorizations for services such as meals, employment, housing, foster care, transportation, and activities of daily living
  • Ensure appropriate referrals and identify accessible providers, specialists, and community resources
  • Maintain accurate documentation in the electronic care management system
  • Ensure compliance with clinical, state, and federal guidelines
  • Provide leadership feedback on quality-of-care and service-delivery improvements
  • Perform other assigned duties and comply with policies and standards

Requirements

What you’ll need
  • Master’s degree in Mental Health Counseling or Social Work
  • 2–4 years of related experience
  • Unrestricted behavioral health professional license required upon application submission, based on state contract requirements (e.g., LCSW, LMFT, LMHC, or LPC)
  • License must be active in the applicable state
  • Strong clinical documentation, problem-solving, communication, attention to detail, organizational, and time management skills
  • Proficient technology skills and direct experience with web-based applications and case management systems
  • Experience with TruCare, Microsoft Outlook, M365 Copilot, Excel, PowerPoint, Word, and OneNote
  • Ability to work independently with minimal supervision
  • Ability to adapt to changing business needs
  • DFPS background check required for Superior Health Plan
  • Preferred: 4+ years of case management experience
  • Preferred experience in child welfare systems, adoption, foster care, Child Protective Services, pediatric acute care, inpatient treatment facilities, schools, or MHA/BHAs for youth
  • Experience collaborating with medical and behavioral health providers and managing community resources for Foster Care Medicaid recipients
  • Experience managing high-volume caseloads
  • Field-based experience strongly preferred
  • Managed care experience is a plus
  • Bilingual English/Spanish skills are a plus but not required
