About the role

Key responsibilities & impact
  • Assess, plan, and implement complex care management activities for pregnant and postpartum members with a history of substance misuse
  • Develop and continuously assess personalized care plans/service plans
  • Collaborate with providers, specialists, community resources, members, and caregivers to address unmet needs
  • Coordinate member care and monitor status, conditions, progress, and care-plan goals
  • Identify barriers and appropriate care-management interventions for escalated cases
  • Review member data, referrals, and intake assessments to improve care quality and operating performance
  • Conduct telephonic, digital, home, and other site outreach
  • Collect, document, and maintain member information and care-management activities for regulatory compliance
  • Educate members and families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits
  • Partner with leadership to improve cost-effective care and quality delivery
  • Precept and support clinical new hires during onboarding, competency checkpoints, coaching, and field-practice preparation
  • Perform other duties as assigned and comply with policies and standards

Requirements

What you’ll need
  • Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
  • 4–6 years of related experience
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required
  • Candidates must reside in Indiana
  • Preference for professional experience related to maternal health and/or substance abuse
  • Availability Monday through Friday, 8:00 a.m. to 5:00 p.m. Eastern
  • Ability to conduct face-to-face visits with members throughout Indiana
