About the role
Key responsibilities & impact- Conduct virtual medication history interviews with patients and families via phone or video
- Perform medication reconciliation at transitions of care, including admission, transfer, and discharge
- Use EHR and external data sources to ensure accurate medication lists
- Identify medication discrepancies and escalate them to pharmacists or providers
- Document medication histories and reconciliation activities in the EHR
- Provide medication education support within technician scope
- Support prior authorization workflows, refill coordination, and medication access barriers
- Collaborate with pharmacists and providers on virtual pharmacotherapy interventions
- Assist with high-risk medication monitoring workflows
- Serve as a Virtual Sitter for inpatients requiring 1:1 monitoring
- Operate and troubleshoot patient monitoring systems and respond to alerts
- Answer calls from patients enrolled in virtual home-based programs
- Assist with scheduling, intake, and virtual rooming for telehealth visits
- Communicate with clinical teams and escalate concerns according to established pathways
- Ensure equipment functionality and coordinate technical support with IT or biomedical teams
- Pack and restock home remote patient monitoring equipment
