About the role

Key responsibilities & impact
  • Fulfill the state-contracted Central Florida Behavioral Health Hub role for children with behavioral health diagnoses
  • Provide care coordination while obtaining credentialing and paneling for therapy services
  • Transition to full-time psychotherapy for children and families once credentialed and paneled
  • Conduct patient and family biopsychosocial assessments and establish goals
  • Reassess patient and family situations and modify plans as needed
  • Address social needs including domestic violence, psychiatric emergencies, child abuse, family disputes, end-of-life and bereavement, Social Drivers of Health, and treatment compliance
  • Participate in interdisciplinary patient and family assessment and treatment
  • Consult with physicians and other healthcare team members regarding psychosocial needs
  • Collaborate with the multidisciplinary team on patients' mental and behavioral health needs
  • Act as liaison between the clinic and community agencies
  • Coordinate and provide therapy sessions, including parent training
  • Document interventions in the Electronic Medical Record
  • Follow Medicaid, CMS, and other regulatory/compliance requirements for documentation and billing
  • Interface with insurance providers, case managers, and service authorization stakeholders
  • Maintain effective working relationships across disciplines
  • Keep current on community resources and provide families with resource-specific information
  • Assist with weekly group parent training sessions

Requirements

What you’ll need
  • Masters degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)
  • Active LCSW licensure required
  • Knowledge of age-specific principles of growth and development
  • Effective communication skills to collaborate with all members of the Central Florida Behavioral Health Hub team
  • Ability to conduct biopsychosocial assessments and establish goals
  • Ability to provide care coordination, psychotherapy, counseling/therapy, parent training, and therapeutic interventions
  • Ability to document interventions in the Electronic Medical Record
  • Knowledge of Medicaid, CMS, and other regulatory/compliance requirements for documentation and billing
  • Ability to interface with insurance providers, case managers, and others involved in service authorization
  • Ability to maintain knowledge of community resources
  • May require working evenings and/or weekends
