About the role

Key responsibilities & impact
  • Coordinate administrative, operational, and customer support activities for care management programs and healthcare services.
  • Perform advanced administrative, operational, and customer support responsibilities independently using judgment.
  • Support assessment, coordination, and facilitation of members' care needs.
  • Collaborate with members, providers, caregivers, and care teams to support member wellbeing.
  • Coordinate care and review and maintain patient records.
  • Prepare care summaries that inform care teams, guide next steps, and improve patient outcomes.
  • Prioritize requests, interpret and adapt procedures, and apply established techniques.
  • Report to the Associate Director, Consumer Experience.

Requirements

What you’ll need
  • High School Diploma or GED.
  • Experience providing in-center or virtual support or interaction with patients.
  • Experience in patient support, care coordination, or a related healthcare setting.
  • Experience with data entry and maintaining information within an electronic medical record (EMR) system.
  • Can work EST or CST.
  • Self-provided internet service with at least 25 Mbps download speed and 10 Mbps upload speed.
  • Wireless, wired cable or DSL connection is suggested.
  • Ability to work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
  • Spanish fluency preferred.
