About the role

Key responsibilities & impact
  • Report to the E-Referral Supervisor
  • Monitor multiple e-referral systems to identify new referrals and manage alerts and notifications
  • Monitor email requests to pull referrals from applicable referral systems
  • Perform follow-up and manage related communication
  • Scan documentation to determine patient eligibility for services, including service area, orders, and insurance
  • Monitor the status of all referrals and follow up throughout the process
  • Report referral status weekly
  • Communicate with branch, sales, and leadership by email or phone regarding status updates and referral decisions
  • Manage the ERST log daily with all referrals and ROCs decisions
  • Maintain acceptable productivity levels according to work assignment and departmental standards
  • Follow regulatory compliance areas, policies and procedures, and company best practices
  • Serve as the initial contact point for clients or referral sources to promote continuity of care
  • Collect and document referral information, manage compliance, and follow up on initial referral processes and decisions

Requirements

What you’ll need
  • High School Diploma or its equivalent
  • Can type 40 words per minute or more
  • Comprehend and apply principles of basic math while analyzing data and generating reports
  • Proficiency with Microsoft Office applications including Word, Excel, Outlook, and Teams
  • Good logical thinking and decision-making skills
  • Can follow strict timelines
  • Communication with all levels of management
  • Can work with multiple screens/systems ensuring accuracy to detail
  • Time management skills and can work in a fast-paced environment
  • Must read, write and speak fluent English
  • Must have good and regular attendance
  • Availability to work 40 hours per week, Monday-Friday 9am-6pm EST
  • Occasional weekend coverage required
  • Must be available to work at least 2 holidays per year, with additional holiday coverage as needed
  • Home internet with at least 25 Mbps download and 10 Mbps upload speed
  • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
  • Occasional travel to Humana offices for training or meetings may be required
  • Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse for candidates applying from a Florida work location
  • Preferred: Experience processing incoming referrals using eReferral portals and EPIC systems
  • Preferred: 1 or more years customer service or other healthcare related experience
  • Preferred: Experience working in a home health environment
  • Preferred: Knowledge of issues related to delivery of home health services
  • Preferred: Working knowledge of Medicare enrollment and guidelines governing home health agencies
  • Preferred: Associates degree
