About the role
Key responsibilities & impact- Lead and support a team of clinicians delivering home-based care
- Oversee clinical operations, patient care delivery, staff management, documentation quality, and regulatory compliance onsite in-office
- Review referrals and determine admission appropriateness
- Assign clinicians and ensure Plans of Care meet patient and agency standards
- Guide, support, and educate clinicians
- Support goal-setting, care planning, and clinical decision-making
- Provide clinical support during operating hours
- Ensure clinical documentation, audits, and billing meet Medicare, payer, and company standards
- Monitor case management quality and outcomes
- Participate in hiring, training, performance evaluation, coaching, and corrective action
- Conduct staff education based on documentation review, utilization review findings, and performance improvement data
- Coordinate communication among physicians, team members, and caregivers
- Support care coordination, discharge planning, and outcome achievement
- Participate in quality improvement, data tracking, budgeting, marketing initiatives, and community relationship development
- Provide limited direct patient care in exceptional or unplanned circumstances
- Act as Branch Director in the Branch Director's absence
- Perform additional tasks supporting clinical operations and organizational goals
Requirements
What you’ll need- Current state license as a Registered Nurse
- Proof of current CPR
- Valid state driver's license
- Auto insurance
- Reliable transportation
- Two years as a Registered Nurse
- At least one year of management experience in a home care, hospice, or equivalent environment
- Required to undergo TB screening if selected
- Must maintain personal vehicle liability insurance meeting state minimum limits or the specified coverage limits, whichever is higher
