About the role

Key responsibilities & impact
  • Provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN
  • Implement individualized care plans with patients, families, and the healthcare team
  • Educate patients and families on disease management, treatment options, and self-care techniques
  • Maintain accurate documentation of patient visits, interventions, and outcomes
  • Communicate promptly with patients, physicians, clinical managers, and other healthcare team members regarding needs and status changes
  • Participate in care coordination and discharge planning
  • Report to the Clinical Manager
  • Support patient recovery, rehabilitation, and independence in the home setting

Requirements

What you’ll need
  • Current nursing license in the practicing state
  • Valid driver's license
  • Auto insurance
  • Reliable transportation
  • Current CPR certification
  • Two years' experience as an LPN/LVN in a clinical setting
  • Nursing experience in a Home Health or Hospice setting (preferred)
  • Must maintain personal vehicle liability insurance in accordance with state minimum requirements or specified higher limits
  • Must complete TB screening if selected
  • Scheduled for 40 hours per week
