About the role
Key responsibilities & impact- Provide skilled nursing care to patients in their homes under the direction of an RN
- Implement individualized care plans with patients, families, and the healthcare team
- Educate patients and families on disease management, treatment options, and self-care techniques
- Maintain accurate documentation of patient visits, interventions, and outcomes
- Communicate promptly with patients, physicians, clinical managers, and healthcare team members regarding needs and status changes
- Participate in care coordination and discharge planning
- Support patients' recovery, rehabilitation, and independence in the home setting
Requirements
What you’ll need- Current nursing license in the practicing state
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years' experience as an LPN/LVN in a clinical setting
- Nursing experience in a Home Health or Hospice setting (preferred)
- Must maintain personal vehicle liability insurance in accordance with state minimum limits or specified Humana limits, whichever is higher
- Patient-facing role requires TB screening if selected
