About the role

Key responsibilities & impact
  • Guide, support, and educate clinicians
  • Assist with goal setting, care planning, and clinical decision-making
  • Provide clinical support during operating hours
  • Ensure clinical documentation, audits, and billing meet Medicare, payer, and company standards
  • Monitor case management quality and outcomes
  • Participate in hiring, training, performance evaluation, coaching, and corrective action for clinical staff
  • Review and audit OASIS, visit notes, and plans of care for accuracy, completeness, and compliance
  • Collaborate with field-based clinicians to ensure timely documentation and resolve deficiencies
  • Assist with OASIS accuracy and validation
  • Monitor outcome-based quality indicators and help develop performance improvement plans
  • Participate in quality improvement projects, internal audits, and regulatory survey readiness
  • Track and report quality metrics, trends, and improvement areas to leadership
  • Provide staff education and support with compliance and documentation standards
  • Stay current on federal and state home health care regulations
  • Provide limited direct patient care in exceptional or unplanned circumstances
  • Report to the Branch Director
  • Participate in rotating on-call shifts

Requirements

What you’ll need
  • Graduate of an accredited School of Nursing
  • Current state license as a Registered Nurse
  • Proof of current CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
  • Minimum of 2 years of experience as a Registered Nurse
  • Traditional home health experience required
  • OASIS experience required
  • Foundational knowledge and basic understanding of CMS PDGM required
  • Management and people leadership experience preferred
  • OASIS certification (HCS-O, COQS, and/or COS-C) preferred
  • Homecare Homebase (HCHB) experience preferred
  • Must complete TB screening if selected
  • Must maintain personal vehicle liability insurance in accordance with state minimums or specified higher limits
