About the role

Key responsibilities & impact

Provide personalized, one-on-one skilled nursing care to patients in their homes
Work under the direction of an RN
Implement individualized care plans with patients, families, and the healthcare team
Educate patients and families on disease management, treatment options, and self-care techniques
Maintain accurate documentation of visits, interventions, and outcomes
Communicate promptly with patients, physicians, clinical managers, and other healthcare team members regarding needs and status changes
Participate in care coordination and discharge planning
Support patients' recovery, rehabilitation, and independence

Requirements

What you’ll need

Current nursing license in the practicing state
Valid driver's license
Auto insurance
Reliable transportation
Current CPR certification
Two years' experience as an LPN/LVN in a clinical setting
Nursing experience in a Home Health or Hospice setting preferred
Must maintain personal vehicle liability insurance in accordance with state minimum limits or specified higher limits
Must complete TB screening if selected
Patient-facing role subject to Humana's driver safety program
